This is a clinic that is held once a day to work-up veterans with
memory impairment. The pharmacist’s role is to evaluate
medication adherence as well medications that could be interfering
with memory and/or function

Levindale Hebrew Geriatric
Center, Baltimore, Maryland (1998 – Present)
This is a clinic that is held once a week to evaluate older individuals with an interdisciplinary team of geriatricians, social worker, psychology, physical therapy, dentistry and pharmacy.

Mellitus: Disease State Management Proposal Levindale Hebrew
Geriatric Center, Baltimore, Maryland (2001 – Present)
This is a disease state management focus for older individuals with diabetes residing in nursing homes. The clinical pharmacist works collaboratively with the physicians, nurses, direct care staff and patients to meet the guidelines for caring for older patients with diabetes. Through education, chart review and clinical consults, the care of older individuals with diabetes will improve.
Hardesty J, Brandt N., Feinberg M. Depression Collaborative in Primary Care. Chase Brexton Health Services, Pikesville, Maryland (2002- Present)
This is a health disparity initiative to identify and treat individuals with depression in primary care. The clinical pharmacist is involved in care plans, monitoring and follow-up of individuals for efficacy of treatments.
Medication Management Program with Baltimore City Commission on Aging, August (2001 - Present)
This is a federally funded local project to provide medication management and educational sessions to seniors and staff in senior housing sites throughout the city of Baltimore. This project will train students to train staff. In addition, educational materials will be developed to assess and further the skills of the participants and staff.
Elder Health Member Medication Review Program (July 2000 - Present)
This program provides clinical pharmacy consulting services on a regular basis to improve adherence and provide pharmaceuticals to a frail elderly population. Clinical Lamy Center faculty, residents and students provide services twice a week to two different clinic sites through Elder Health Inc. This is an ongoing partnership that strengthens the voice of senior care pharmacy in community clinical settings.
