Below are tools and resources you may need in planning your activity and submitting your application.

Activity Announcement Guidance Document: Activity announcements and marketing/promotional materials must include specific information per ACPE policy. Use the Guidance Document as a checklist when creating your activity announcement.

Attendance Sheet Template: Use this as a guide for your attendance sign-in sheet if you do not already have a template. You may update this template to meet the needs of your specific activity. If possible, we recommend that you pre-type the names in the ‘Name’ column to ensure legibility. All attendees must sign-in to your activity in order to receive CE credit. You must provide verification of attendance for those completing the activity virtually (i.e., webinar attendees).

Activity Timeline Document: This table provides an overview of deadlines and important dates during the activity planning process, application submission, as well as pre- and post-activity dates.

Conflict of Interest Disclosure Form: The Conflict of Interest Disclosure form must be completed by all persons in a position to control the content of the CE activity. All relevant financial relationships must be declared. Forms can be submitted during the application process or emailed to continuinged@rx.umaryland.edu.

Faculty/Speaker Guidance Document: The Faculty/Speaker Guidance Document contains information regarding the development and writing of learning objectives.

Home Study Hour Justification Template: For home study activities, Joint Provider are required to submit written justification for the amount of CE Contact Hours of an activity. Use this template to submit the justification to the Office of Continuing Education. This template also contains links to a Workload Estimator you can use to calculate the time required for certain activities.

Joint Provider Guidance Document: The Joint Provider Guidance Document outlines the process for a CE activity from the initial planning through final invoicing and the role of the Joint Provider Planning Committee.

Module Development Guidance Document: The Module Development Guidance Document is an outline of the process used by the CE Office to create and develop online, enduring modules. You may use this as a guide during your activity development process.

Pharmacy Keywords: In order to create an ADF, ACPE requires keywords related to the activity content. The Pharmacy Keywords is the full list of ACPE-approved keywords.
