Age-Friendly Regional Champions Resources

The Institute for Healthcare Improvement (IHI) provides resources for Age-Friendly Health Systems in coordination with the John A. Hartford Foundation. This initiative is grounded in the 4Ms Framework: What Matters, Medication, Mentation, and Mobility. This movement works to integrate evidence-based practices to align care with the health goals and needs of older adults.

Age-Friendly Health Systems Recognition

The IHI recognizes health care organizations that are committed to delivering evidence-based care for older adults using the 4Ms framework.

The two levels of recognition are:

  • Level 1 (Participant)
Level 1 teams have completed a 4Ms Care Description to outline how it will assess, document, and act on all 4Ms at its care setting.
  • Level 2 (Committed to Care Excellence)
Level 2 teams have achieved Level 1 and have submitted at least three months of counts of the number of older adults who received care that included all 4Ms.
